Overnight Facility Maintenance Report Instructions

MATC Maintainers: (Here's your requested email-able Overnight Facility Maintenance Report form.)
   First, just fill in the
Work Trip Report form with your field data and press the SUBMIT button on the form. Then fill out this form if you maintain a lean-to or campsite and press the SUBMIT button on the form. Both forms will be processed by a CGI program form handler and routed to myself, the MATC webmaster. I will then forward the reports to your overseer.
    Fill in the name of your facility, the date of your site visit, and your name.
    Under Signing, Lean-to &/Or Campsite, and Privy, check the box if each is present on site (Yes=present   No=not present).
    For the second section, list your comments and recommendations in the appropriate slots. If your privy is full and needs to be relocated to a new hole, contact your overseer or indicate that on the form, so he or she can schedule a site trip and round up additional helpers.
    I put the district overseers' names up for new maintainers who forgot or don't know their overseer's name yet. If you don't know who your District Overseer is, here they are:

Check site for overall cleanliness and carry-out all trash

Check area for signs of environmental damage that needs attention
